Registration For Premier Physician Network
Premier Physician Network offers you one of the largest selections of doctors and advanced practice providers to choose from in Southwest Ohio. More than 650 primary and advanced practice providers see patients in 125 centers, from Butler and Warren counties in the south to Darke and Miami counties in the north. Call your doctor’s office directly to schedule an appointment. Many PPN physicians are accepting new patients.
If You’re a New Patient
Please bring the following to your first appointment:
- Medications or a list of the medications you are currently taking
- A list of known allergies
- Photo identification
- Health insurance card(s)
- Patient registration forms (if you’ve filled them out in advance)
For patient safety, your photo will be taken at the reception desk for inclusion in your electronic medical record.
You will also be asked to sign up for MyChart if you don't already have a Premier account, giving some of the benefits like scheduling appointments, seeing test results, and paying your bills online.
At your first appointment, you may be asked to complete a Patient Registration Form and a medical history form. You may download these and fill them out in advance. For questions, please refer to your doctor's website, accessible through the Premier Physician Network Provider directory.
You may also be asked to complete a Medical Records Release Form, which allows your new physician’s office to request medical records from your previous doctor.
If You’re a Returning Patient
Please bring your photo ID and your insurance card to each appointment so we can keep your information up-to-date. Please notify the office of any changes to your address, phone number, email address, insurance and financial information, or emergency contacts.
All patients should be prepared to pay for co-pays, deductibles and coinsurances at the time of service.
