MyChart® is an electronic health record that offers you personalized and secure online access to portions of your medical records. This free service enables you to securely log in from any computer or smart device, any time, to track your health, schedule appointments, view test results, request medication refills, and so much more. Manage Your Health
With MyChart, you can easily schedule, reschedule, or cancel an appointment online with one of your existing care providers. You’ll be able to view details about your upcoming appointments, including any prior instructions or travel directions, and you can also choose to receive appointment reminders ahead of time to help you stay on track.

Same-Day New Problem Visits
Same-day appointments are available to schedule in MyChart first thing every morning for existing patients who need an acute care visit.
Schedule a Video Visit
Many providers offer the option of a video visit for existing patients for non-urgent symptoms. You must be in the state of Ohio at the time of your visit. Conveniently meet with your provider in the comfort of your home via smart phone, tablet, or computer.
Schedule an Appointment with a New Provider
If you need to schedule an appointment with a new provider, use our Find a Doctor site to find a new provider and schedule your visit. If the provider you selected has new patient online scheduling available, you will see their availability and can begin the scheduling process. You will be prompted to sign into your MyChart account to finalize your appointment time. Once your appointment is scheduled, it can be managed right in MyChart.
Schedule Your Physical Therapy Consultation
If your primary care provider or specialist has referred you to physical therapy, you can easily schedule your initial consultation online with the provider of your choice by using our Find Your PT tool. If you know the specific type of physical therapy you need, you can search providers who specialize in different types of physical therapy. During the scheduling process, you will be prompted to sign into your MyChart account to finalize your appointment time without having to enter extra personal information.
Schedule Your Screening Mammogram
Women aged 40 and above can schedule their annual screening mammogram online without a physician referral. MyChart allows you to schedule an appointment at any of Premier Health’s breast imaging locations.
Ticket Scheduling in MyChart
For tests or procedures requiring a referral from your provider, a scheduling ticket may be issued in MyChart. This feature enables you to conveniently schedule the service directly within MyChart, offering an efficient alternative to contacting Central Scheduling.
On-Demand MyChart Virtual Urgent Care Visits
Premier Health offers Virtual Urgent Care from 9 a.m. to 9 p.m., through MyChart. You never need an appointment, and connecting is as easy as using your smartphone, tablet, or computer. Our Premier Health Urgent Care provider will ask you questions about your symptoms and recommend steps you can take, or refer you for in-person diagnosis and treatment.
MyChart E-Visits
E-Visits are a convenient alternative to scheduling an appointment for certain symptoms or issues. Although e-visits are interactive, they don’t occur in real time, and there is no video component. You’ll spend about 10 minutes answering questions about your symptoms. A physician or advanced practice provider will review your symptoms and medical history and respond to let you know a treatment plan or to schedule an in-office visit.
Connect with your care team by sending secure, non-urgent medical questions and messages through your MyChart Message Center. You can ask questions about test results, clarify information from your office visit, update your medical information, initiate a referral request, and more.
Please allow your provider up to 48 hours to respond to your message.

You can quickly and easily request prescription refills online anytime through MyChart, whenever it’s most convenient for you.
Some prescriptions will require you to be seen by your provider to get it renewed.

Looking for your lab or test results? You can view them in MyChart, along with any past visit summaries, current medications, list of allergies, and other important health information.
Download Your Health Summary
To download your health summary through MyChart:
- In the navigation bar on the left, choose My Medical Record.
- Choose Download Summary.
- Download your health summary, with the option of adding password protection.
Correct Or Update Your Health Information
Your MyChart information comes directly from your health care provider's office, and from your Premier Health hospital, using Premier Health's electronic health record. Your health information is reviewed and updated in your electronic medical record at each visit. To correct any inaccurate information, ask your provider to make any needed changes at your next visit. You may also send a message to your provider in MyChart to request a correction.
If the inaccurate information is related to a hospital visit, hospital stay, or an emergency department visit, you should contact the health information (medical records) department at the hospital to make corrections. You may be asked to complete a Request for Amendment form.

Records From a Non-Premier Health Provider
If you are seen at, or by, a non-Premier Health facility or physician, contact their medical records department to obtain a copy of your medical records. These can be taken to your Premier Health primary care physician. If you are seen by a physician who uses Premier Health's electronic health record, you can also add missing medications and allergies in MyChart. The information will have to be made a permanent part of the record during a physician office visit.
MyChart: Share Everywhere
Your provider might request that you record information such as your daily glucose or blood pressure readings in your MyChart® account. When you enter the information in the Track My Health feature, your provider automatically receives the readings so he or she can monitor your progress between visits.
To record the initial reading, you must use an internet browser (not the MyChart mobile app). For all future daily recordings, you can use the MyChart app or an internet browser.
- Go to Messaging > Message Center. Open the message from your physician about a new flowsheet. The flowsheet is what you use to enter your readings in the Track My Health feature.
- Open Track My Health by clicking the link in the message.
- Click the flowsheet’s name to open it, and then click Add New Data to start recording your readings.
- Enter your reading with the date and time and click Continue.
- Verify that you have entered your reading correctly and click Submit.
- If you need to change or remove a reading after you’ve submitted it, you can do so by clicking Edit or Delete.
To track your daily health readings over time in a table or graph:
- On the Track My Health page, click a flowsheet’s name to view previous readings in a table.
- From this page, you can:
- Change the orientation of the table. Click the More Options link to choose whether dates appear as rows or columns.
- View the readings in a graph. Click Graph to change the display.
- Customize the date range or the number of readings that appear. Change the From and To fields or the latest values field and click Apply to update the display.
Preventive care procedures are suggested based on your age and gender and, in some cases, a specific diagnosis you may have. With MyChart, you can set reminders for these visits so you can easily stay on top of your health.
During your next visit, discuss your health information with your doctor, who can override preventive care recommendations that may not apply to your specific situation. If you see an inaccurate date for a procedure, please discuss it with your doctor so your record can be updated.
To be recorded in MyChart, your preventive care needs to be done by a health care provider who uses Premier Health’s electronic health record. If your provider doesn’t use Premier Health’s electronic health record, your procedures or tests done in the office will not be reflected in MyChart.
