You can access your medical records 24/7 using your MyChart account.

Or use our convenient portal to request records from any Premier Physician Network office.

Premier Health offers financial assistance for patients with limited resources and insufficient insurance coverage.

Human Resources

Experian provides proof of employment and income for current and former employees of Premier Health. Any lenders, pre-employment screening firms, property managers or social service/government agencies who may need verification of employment may visit  www.experianverify.com. If wage information is needed, the employee or former employee must sign an authorization form provided by Experian.   

This service is provided at no cost to employees, former employees, or Premier Health.

If you already have a Premier Health MyChart account established and are having trouble accessing your account, please call MyChart Technical Support at (866) 668-0592(866) 668-0592.

Registering for Premier Health MyChart® is easy. Request a MyChart activation code during your next appointment with a Premier Physician Network health care provider or at a Premier Health facility. You can also request an activation code by calling your provider’s office or by going to the MyChart login page.

Use your activation code to activate your account from a computer or smart device of your choice. Simply go to the MyChart activation page, enter the code, and get started.

If you don’t already have a Premier Physician Network provider, find one near you using the Find a Doctor search tool.
