Register For MyChart

Registering for Premier Health MyChart^® is easy. Request a MyChart activation code during your next appointment with a Premier Physician Network health care provider or at a Premier Health facility. You can also request an activation code by calling your provider’s office or by going to the MyChart login page.

Use your activation code to activate your account from a computer or smart devices of your choice. Simply go to the MyChart activation page, enter the code and get started.

If you don’t already have a Premier Physician Network provider, find one near you using the online search tool.

Family Medical Information

Can you access the medical records of a family member on MyChart? 

Yes, under some circumstances. This is called proxy access and allows a parent (or guardian) to log into their personal MyChart account, then connect to information regarding their family member. Complete a Proxy Consent Form and return it to your physician's office to request access to this convenient service. 

Parents may have proxy access to their children's accounts until they reach the age of 12. After that, a child's account become their own and they may elect to share it with their parent or guardian at their discretion. 

Parents may have access to a minor child's (over the age of 12) account or another adult's account if that person is incapacitated and unable to manage their own affairs. Circumstances that may justify authorization include mental disability, dementia, or other health or mental issues that could compromise your family member’s judgment or ability to completely understand and make decisions about their health. The parent or guardian will need to have legal guardianship or power of attorney for the individual for whom they are seeking proxy access, or the individual will need to sign an authorization allowing access.

Due to the sensitive nature of medical information, each adult in your family must sign and submit a Release of Information request and establish their own MyChart account.
