Application Process

Applicants for Miami Valley Hospital's PGY1 Pharmacy Residency Program should be a citizen of the United Sates, a graduate or candidate to graduate from an ACPE accredited degree program (or one in the process of pursuing accreditation), or have a Foreign Pharmacy Graduate Examination Committee (FPGEC) certificate from the NABP.

Please submit application using the ASHP supported Pharmacy Online Residency Centralized Application Service (PhORCAS). The following items must be received by January 2 to complete this application:

• Letter of Intent
• Curriculum Vitae
• Copy of college transcript(s)
• Three letters of recommendation (utilizing PhORCAS format)

We highly recommend that one to two of the letters of recommendation be from a clinical rotation within the past 6 months.
