This month we are featuring information about Fidelity Health Care as a valuable service that can often improve quality and lower the total cost of care when appropriate, as well as a description of the benefits of a Clinically Integrated Network. We’ve also provided some helpful guidelines for utilizing Transitional Care Management codes and service.
