At Premier Orthopedic Spine Center, we consider it a privilege to provide you with patient-centered care that makes a positive impact on your health. You can depend on us to give you the time, attention and care you deserve.
We use advanced forms of non-surgical treatment first and consider surgery only when necessary. We listen to your concerns and partner with you to develop your personalized treatment plan.
We are currently accepting new patients. Please have your primary care physician send a referral with current imaging and office visit notes to (513) 420-3773(513) 420-3773.
Appointment/Procedure Preparation
We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below:
What to Expect
To ensure a timely visit, we request that you complete your Neck or Back Forms before arriving to your appointment:
- Low Back Pain Disability Questionnaire
- New Back Questionnaire
- Neck Pain Disability Questionnaire
- New Neck Questionnaire
- Visual Pain Scale
We also request that you complete any prescribed testing prior to your visit.
Co-pays and deductibles are due at the time of service.
What to Bring to Your Appointment
- Completed forms
- Co-pay and deductible
- Insurance cards
- Medication list
- Photo ID
After Your Appointment
At the end of your appointment, you will be given an After Visit Summary that details everything discussed during your visit. It will also list any procedures, testing or lab work that is required before your next appointment. Please bring your completed Neck or Back Forms to each visit.
