Patient Guide

Premier Acute Care Surgery - Apple Street is currently accepting new patients and will see you immediately, usually within 48 hours of the initial call or referral. To schedule an appointment, please call (937) 208-5055(937) 208-5055.

Any special instructions regarding your appointment will be discussed with you during the initial phone conversation.

What to Expect

If you are a new patient, please be sure to bring either your medications or a list of the medications you are currently taking along with your insurance card(s). Please bring your insurance card and photo identification to your appointment. Established patients should bring new insurance card(s) when applicable

All patients should come prepared to pay any co-pays, deductibles, or co-insurances at the time of their visit.

After Your Visit or Procedure

After your visit, you will receive instructions for making a follow-up appointment. If needed, you will also receive information about prescriptions, blood work, or other orders.

You also will be given the opportunity to sign up for MyChart – an electronic medical record which allows you to review your medical information online and communicate with the team at Premier Acute Care Surgery - Apple Street through the secure website. MyChart also allows you to schedule appointments, check your current and past lab results and ask other medical questions. If tests have been ordered, you will receive a follow-up phone call or a letter with your results.

Billing and Insurance
