The skilled physicians and experienced providers at Upper Valley Family Medicine are committed to providing you comprehensive care in a professional, welcoming environment. Our facilities are equipped with a lab draw station, and we offer imaging services and occupational and physical therapy.

Appointment/Procedure Preparation

Any special instructions regarding your appointment will be discussed with you during the initial phone conversation.

What to Expect

If you are a new patient, please be sure to bring your medications to your appointment as well as a list of allergies, a photo ID, and your insurance card(s). Established patients should bring their insurance cards to each visit. You will be asked to complete a new patient registration form annually so that your information is updated and you renew permission to bill your insurance.

All patients should come prepared to pay any co-pays, deductibles, or co-insurances at the time of their visit.

After Your Visit or Procedure

At the end of your visit we will let you know if you require follow-up care with another provider within the Premier Health network. If the physician ordered additional tests or lab work for you, our staff is happy to schedule these for you or give you the information to make the appointments yourself.

Our office communicates lab and X-ray results promptly either by phone, letter, or MyChart.
