Payment Options
Payment is due upon receipt of your statement. You may pay your account using debit or credit (Visa, MasterCard, Discover, and American Express are accepted).
Premier Health Hospitals*, Premier Physician Network (PPN), and Premier Health Urgent Care Payment Options
In MyChart. Your Premier Health hospital, PPN providers, and urgent care bills are available to pay. MyChart also provides the ability to pay as a guest.
By phone. To make a credit card payment, call Premier Health Patient Billing at (937) 208-5555(937) 208-5555, Monday through Friday, 8 a.m. to 6 p.m. Please have your statement nearby when you call.
In person. Credit card payments are accepted at each hospital's cashier's office or at your Premier Physician Network practice.
By mail. Detach the bottom portion of your billing statement and include it with your payment in the envelope provided.
Payment plan. As a service to our patients, Premier Health offers payment plans for those who are unable to pay their balance in full. There are two ways to set up a payment plan.

Online. Pay your bill through one of our secure, online payment portals.

Samaritan Behavioral Health Payment Options
