Appointment/Procedure Preparation

New Lebanon Family Care is currently accepting new patients. To schedule an appointment, please call us at (937) 687-1331(937) 687-1331.

Any special instructions, such as fasting before a blood draw, will be discussed with you at the time you schedule the appointment.

We ask new patients to arrive 15 minutes prior to their initial appointment to complete any of the necessary forms from the list below: 

• Patient Registration Form - Adult
• Patient Registration Form - Minor
• Established Patient History Form
• Medical Release Form
• Verbal Communications Form
• Advance Directives Patient Education Form
• Advance Directives - Health Care Power of Attorney and Living Will

What to Expect

Please be sure to bring your insurance card(s) and a list of your current medications. We will also be asking you to sign up for MyChart. Please bring your e-mail address with you.

After Your Visit or Procedure

Your physician may request a follow-up visit. If blood work is done during your visit with us, you will receive a results postcard in the mail within two weeks. If you sign up for MyChart, the test results will be released to your MyChart account within 3 days of being received and imaging results will be released in 5 days.
