Home Care Elite Top 500

Fidelity Health Care was named to the Top 500 of the 2019 HomeCare Elite, the 12th consecutive year Premier Health’s home health agency has been recognized on the list of top-performing home health agencies in the United States. HomeCare Elite compiles the most successful Medicare-certified home health providers in the country. This annual review identifies top agencies by measuring performance in quality outcomes, implementation of best practices, patient experience, quality improvement and consistency, and financial health.

Home Health Compare

In 2025, Fidelity Health Care received a 5-Star Rating for quality of its patient care from Home Health Compare, a ratings system operated by the Centers for Medicare & Medicaid Services (CMS). The 5-Star Rating from Home Health Compare, the highest possible, places Fidelity Health Care in the top 6 percent of home health care agencies across the country, and in the top 3 percent in Ohio. Star ratings range from 1 to 5 and are based on eight measures of quality that give an overview of performance. A star rating higher than 3.5 means that an agency performed better than average compared to other agencies.

Medicare And The Joint Commission

Fidelity Health Care is Medicare certified, and we have been accredited by The Joint Commission in all of our service areas: home health care, home infusion, durable medical equipment, oxygen and respiratory equipment, clinical respiratory, and independent care services.
