Appointment Preparation

Please call between 10 am and 3 pm Monday, Thursday, or Friday to speak with office staff about becoming a new patient.

We ask all patients to arrive 15 minutes prior to their appointment to complete any of the necessary forms from the list below: 

• Patient Registration Form - Adult
• Patient Registration Form - Minor
• Medical Release Form
• Verbal Communications Form
• Advance Directives Patient Education Form
• Advance Directives - Health Care Power of Attorney and Living Will

What to Expect

You need to bring your insurance card, photo identification and a list of medications. All patients are required to update their registration form annually. Copays are due at the time of service.

After Your Visit

You will be provided with an After Visit Summary at the end of each appointment that will summarize your visit, list any medication changes and note the date and time of your future appointment.
