Access Medical History and Medication Lists on MyChart

From My Chart you can access your own personal medical record. My Chart is a secure on-line portal for your medical information. From you’re My Chart “My Medical Record” tab, you can see several categories with your personal health history; including test results, medication lists, lists of allergies, immunizations, preventative care that you’ve had or need, and medical history. MyChart is a great tool to partner with your doctor in your own healthcare.
